The Evolution Dental team is pleased to offer our Annual Membership Plan as an alternative to dental insurance.
Adult Membership (13 and Older) – $55 per month
Perio Membership (Periodontal Disease) – $89 per month
*The agreement will automatically renew each year on the anniversary date, unless either party cancels by giving 30 days cancellation notice. Some restrictions apply. Cannot combine with insurance.
What’s Included in the Plan?
Our Annual Membership Plan includes:
- Professional dental cleanings (2 per year)
- Doctor exams (1-2 per year)
- Oral cancer screening
- Periodontal screenings
- Needed x-rays*
- One emergency exam per year (if needed)
- 15% off major treatments (crowns, veneers, periodontics, root canals, dentures) savings up to $1500 total annually.
- 20% off basic treatments (dental sealants, fillings, core build-up, oral surgery [limited], night guards)
